JOB SUMMARY Responsible for the operations of the Vicar's Landing Home Health Agency. Markets the agency to members of Vicar's Landing, and other areas, as licensed. Participates in the development and implementation of the organization’s strategic plan. Responsible for the quality, planning, marketing, and financial management of the Home Health Agency. ESSENTIAL DUTIES & RESPONSIBILITIES 1. Provides leadership for the Vicar's Landing Home Health Agency. 2. Plan, organize, direct, and evaluate operations to ensure the provision of adequate and appropriate care and services. 3. Generating referrals for home health by building relationships with physicians, long term care, independent and assisted living facilities, and other community resources. 4. Conduct sales calls and evaluate results and effectiveness of sales activity. 5. Support business development activities and help establish strong relationships with new and existing referral sources. 6. Ensure agency follows all applicable federal, state, and local laws and regulatory agencies. 7. Be responsible for fiscal planning, budgeting, and management of operations in accordance with established fiscal parameters. 8. Recruit, employ and retain qualified personnel to maintain appropriate staffing levels. 9. Establish and maintain effective channels of communication. 10. Assure skilled nursing and other therapeutic services furnished are under the supervision of a physician or RN. 11. Assure appropriate staff supervision during all operating hours. 12. Ensure completion, maintenance, and submission of required reports. 13. Ensure documentation of services is accurate and timely. 14. Responsible for supervision and evaluation of satisfaction surveys on all individuals served. 15. Charts progress notes, reviews client status and care provided. Provides progress notes to the member's physician about care when the patient's conditions change or there is a deviation from the plan of care. 16. Directs the activities of clinic and agency staff. Prepares written instructions for the care and services provided in the home, as per state/federal regulations. 17. Demonstrates respect for member confidentiality and member rights. 18. Acts as coordinator of the health care team to maintain continuum of care with all disciplines; including contracted services. 19. Complies with attendance policy and adheres to established professional dress code. 20. Promotes own professional growth by maintaining certifications and by obtaining required CEUs. 21. Demonstrates a thorough knowledge of and practices within the infection control policies and procedures established by the facility. 22. Communicates regularly with Health Services leadership. 23.
