About The Position

At Mass General Brigham (MGB), we are developing an integrated healthcare system for the future. MGB, the largest healthcare system in the state of Massachusetts, has two academic hospitals, three specialty hospitals, seven community hospitals, a health insurance provider (MGB Health Plan), and a Home Care division. The West Health Accelerator is a care model redesign program that seeks to improve care for hospitalized patients within MGB while creating a replicable model for scaling across small community hospitals to regional medical centers through large academic health systems. The MGH (Mass General Hospital) Director of Geriatric Practice, in partnership with program leadership, is responsible for the direction and alignment of activities undertaken by the West Health Accelerator (WHA) team and, in collaboration with leaders and stakeholders, will lead nursing implementation at Mass General Hospital. The West Health Accelerator Program will expand the Superior Treatment of Elders Pathway (STEP) from the Center for Geriatric Surgery (CGS) at Brigham and Women’s Hospital to nine hospitals across Mass General Brigham (MGB) and package the learnings into a scalable national model. The MGH Director of Geriatric Practice will participate in the development, refinement, communication, and actualization of the WHA programs specific to the care of older adults and other adults with frailty within MGH, departments, service lines, disciplines, and role types. This Advanced Practice Provider position will provide high-level oversight, interprofessional collaboration, and direct influence on both clinical care and system redesign — while anchoring these efforts within the division of Palliative Care and Geriatric Medicine, thus ensuring a fuller integration of this work and its long-term sustainability.

Requirements

  • Current licensure as an APRN/Nurse Practitioner in the Commonwealth of Massachusetts.
  • Master’s degree (MSN), Doctorate in Nursing Practice (DNP), PhD in nursing science, or a related advanced clinical degree required.
  • At least 5-7 years of advanced practice nursing experience required, preferably in geriatrics or an equivalent clinical field.

Nice To Haves

  • Certification in geriatric care or related specialties preferred.
  • Minimum of 3-5 years of experience working in healthcare administration, leadership, or program management preferred
  • Experience in quality/process improvement initiatives or healthcare research is preferred.
  • Previous experience in a complex healthcare organization with interdisciplinary collaboration is highly desirable.

Responsibilities

  • Participate in formulating the vision and strategic plan of geriatric efforts within the West Health Accelerator.
  • In collaboration with MGH and WHA geriatric leadership, develop and maintain a comprehensive, effective geriatric care program that aligns with geriatric best practices and meets external standards including that of state and national programs.
  • Collaborate with leadership and frontline providers to integrate geriatric principles into clinical workflows/pathways, patient care guidelines/protocols, and staff development activities.
  • As clinical expert and resource in geriatric care, provide education, mentorship, and clinical coaching to interprofessional teams across multiple units within the academic medical center. Promote the implementation of evidence-based practices to optimize care for older adults, support staff in complex case management, and contribute to quality improvement initiatives. When more focused, or complex clinical consultation is required, facilitate appropriate formal referrals to geriatrics, neurology, or psychiatry.
  • As an advanced practice provider, when on clinical rotation with the Geriatrics Inpatient Team, provide direct consultative clinical services to hospitalized patients, applying advanced knowledge in geriatric care and clinical expertise to support complex patient cases, ensuring optimal care plans are developed and implemented for older frail adults; contribute to the clinical education and mentorship of other healthcare providers and clinical staff, sharing expertise on the care of geriatric populations.
