Requirements

  • Active RN License (Connecticut)
  • Minimum 2+ years Medicare-certified Home Health experience
  • At least 1+ year in a supervisory/clinical management role
  • Demonstrated OASIS proficiency
  • Strong knowledge of: Medicare Conditions of Participation, PDGM, State regulatory standards, Survey preparedness
  • Current CPR
  • TB screening (within 12 months)
  • Physical exam within 6 months prior to patient contact
  • Valid driver’s license & automobile insurance
