Requirements

Active RN License (Connecticut)
Minimum 2+ years Medicare-certified Home Health experience
At least 1+ year in a supervisory/clinical management role
Demonstrated OASIS proficiency
Strong knowledge of: Medicare Conditions of Participation, PDGM, State regulatory standards, Survey preparedness
Current CPR
TB screening (within 12 months)
Physical exam within 6 months prior to patient contact
Valid driver’s license & automobile insurance
