About The Position

Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking a Director of Case Management for a permanent leadership opportunity in Panama City, Florida. The Director of Case Management provides strategic and operational leadership for hospital-based case management and discharge planning services. This role oversees care coordination, utilization management, staffing, and performance improvement initiatives to ensure high-quality, compliant, and patient-centered outcomes across the continuum of care.

Requirements

  • Active, unrestricted professional license for the position you are applying for.
  • Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
  • Minimum of 3 years of recent (within the last year) acute hospital case management experience required.
  • Minimum of 2 years of leadership experience required.
  • Hospital discharge planning experience required.
  • Bachelor’s degree required.

Nice To Haves

  • Master’s degree in Nursing, Health Administration, or Business Administration preferred.
  • CCM or ACM certification preferred.
  • Strong leadership, operational, and performance improvement skills.

Responsibilities

  • Direct and evaluate departmental operations, including the case management model, staffing plans, onboarding, competencies, and use of information technology.
  • Allocate resources effectively to meet productivity, quality, and performance goals.
  • Provide oversight of core case management functions and professional practice standards.
  • Ensure adherence to care coordination and discharge planning processes, including timeliness, quality, and documentation.
  • Collaborate with executive leadership, including finance and division case management leaders, to achieve established goals.
  • Monitor departmental performance through data analysis and implement sustainable performance improvement initiatives.
  • Ensure compliance with policies, procedures, and standards of practice.
  • Establish and maintain strong working relationships with clinical, operational, compliance, and legal stakeholders.
  • Practice and uphold the organization’s Code of Conduct, mission, and values.
