About The Position

Jo.W Staffing Solutions, in partnership with Purple Cow Recruitment, is seeking a highly qualified Director of Case Management. The Director of Case Management is responsible for the overall leadership, management, and coordination of the hospital’s Case Management Department. This role ensures compliance with organizational policies, regulatory requirements, and best practices in patient care coordination and discharge planning.

The Director provides strategic and operational oversight of daily case management functions, collaborates with interdisciplinary teams, and partners with internal departments and external agencies to ensure continuity of care, optimal resource utilization, and achievement of established clinical and operational outcomes. This position requires strong executive presence, proven leadership capabilities, and the ability to motivate teams while delivering measurable results.

Requirements

• Bachelor’s degree (required)
• Active licensure as one of the following (required): Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW)
• Minimum 3 years of acute care hospital case management experience (required)
• Minimum 2 years of leadership experience in acute care case management (required)
• Hospital discharge planning experience (required)

Nice To Haves

• Master’s degree in Nursing, Health Administration, or Business Administration Preferred

Responsibilities

• Ensure effective discharge planning and care coordination across the continuum of care
• Maintain compliance with hospital policies and external regulatory agencies
• Lead, mentor, and develop case management staff
• Collaborate with interdisciplinary clinical teams and external stakeholders
• Monitor department performance metrics and drive quality improvement initiatives
• Ensure optimal utilization of clinical and financial resources
• Support hospital leadership through clear communication and professional presentation
