Our Director of Care Transitions / Social Services Director plays a vital role in our resident and guest outcomes and overall experience. We are looking for a skilled Director of Care Transitions / Social Services Director to become a part of our facility's compassionate and hospitable care-giving team.

Requirements

• At least one year experience working with elderly population in acute, long-term care, or community-based setting, required
• Must be able to read, write, speak, and understand the English language.
• Must be able to make independent decisions when circumstances warrant such action.
• Compassionate and caring demeanor
• Ability to build rapport with residents/families
• Ability to have one-on-one conversations, routine visits with residents, and facilitating difficult conversations with families
• Demonstrate progressive leadership and initiative
• Required to sit, stand, bend and walk regularly; lift and/or move up to 25 pounds.
• Visual and auditory ability sufficient for written and verbal communication.
• The noise level in the work environment is usually moderate.
