About The Position

The Director of Nursing Practice Improvement (PCMH & Quality Outcomes) provides systemwide leadership for Patient-Centered Medical Home (PCMH) implementation, nursing practice standardization, and ambulatory clinic workflow improvement across all clinical sites. Reporting directly to the Director of Nursing, this role translates PCMH standards, quality priorities, and organizational goals into consistent, high-performing nursing and clinic practices. This position ensures delivery of excellent, patient-centered care through high-functioning, team-based care models, using standardized workflows, PERCs-based clinical practices, and continuous training. The Director focuses on practice improvement, education, workflow design, and operational execution, serving as a key strategic and operational partner to the DON. The role collaborates closely with clinic leadership, quality, and operations teams to improve access, efficiency, care coordination, patient experience, and quality outcomes in an FQHC environment. Final clinical nursing authority remains with the Director of Nursing.

Requirements

  • Master’s degree in healthcare administration, Business Administration, Public Health, Nursing Leadership, or related field.
  • Minimum of 4 years of experience in ambulatory practice, including nursing practice and leadership responsibilities.
  • Demonstrated experience with Patient-Centered Medical Home (PCMH) implementation or sustainability.
  • Experience working in a Federally Qualified Health Center (FQHC) or comparable community-based primary care setting.
  • Strong understanding of ambulatory nursing workflows and team-based care models.
  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license, in good standing
  • Level 3 – Requires a high level of concentration and high level of attention intermittently. Generally, the approach to tasks may be consistent, but the number of steps required and/or the number of variables involved creates the possibility of errors unless the incumbent pays close attention.

Nice To Haves

  • Experience supporting PCMH recognition or re-recognition processes.
  • Formal training in quality improvement or process improvement methodologies (Lean, PDSA, Six Sigma).
  • Multi-site clinic experience.
  • Experience designing and delivering staff education and training programs

Responsibilities

  • Lead nursing- and clinic-focused implementation, sustainability, and continuous improvement of PCMH standards across all sites.
  • Translate PCMH requirements into standardized nursing workflows, clinic processes, and team-based care models.
  • Develop, implement, and maintain PCMH tools, workflows, templates, and SOPs.
  • Conduct PCMH readiness assessments and practice-level evaluations.
  • Support documentation and evidence preparation for PCMH recognition and re-recognition in partnership with Quality.
  • Train clinical staff on PERCs (Patient Engagement, Reliability, Communication, and Safety) based clinical practices to ensure consistent, reliable care delivery.
  • Standardize use of standing orders, rooming protocols, care gap workflows, and escalating care processes aligned with PERCs principles.
  • Reinforce reliability behaviors such as structured clinical flow, role clarity, proactive risk identification, and follow-through.
  • Partner with the DON to support staff competency, skills validation, and ongoing reinforcement of PERCs-aligned nursing practice.
  • Ensure excellent patient care by developing and sustaining high-functioning care teams.
  • Promote role clarity, accountability, and interdisciplinary collaboration among LPNs, RNs, MAs, providers, and support staff.
  • Reinforce team-based behaviors that support safety, continuity, patient trust, and positive care experiences.
  • Design and deliver systemwide training on PCMH standards, PERCs-based workflows, nursing practice standards, and clinic best practices.
  • Support onboarding and ongoing education for LPNs, RNs, MAs, and clinic teams.
  • Provide hands-on coaching, real-time feedback, and implementation support to clinics.
  • Reinforce consistent expectations for team performance and patient-centered care delivery.
  • Partner with the Director of Quality to translate data into actionable nursing practice improvements.
  • Support clinic-level performance improvement initiatives.
  • Serve as a key operational and strategic support to the Director of Nursing, including special projects, workflow standardization, and nursing practice reinforcement.

  • Assist with nursing leadership initiatives, cross-clinic standardization efforts, and special projects as assigned by the DON.
  • Act as a liaison between nursing leadership, clinic teams, quality, and operations to ensure alignment and follow-through.
  • Standardize ambulatory nursing and clinic practices across sites to ensure consistency, efficiency, and quality.
  • Lead workflow redesign initiatives focused on access, cycle time, continuity of care, care coordination, and patient experience.
  • Support adoption of evidence-based ambulatory nursing practices aligned with PCMH principles.
  • Serve as a resource to clinic leadership and staff in addressing nursing and clinic operational challenges.
  • Design and deliver training programs related to PCMH standards, ambulatory nursing workflows, and clinic best practices.
  • Support onboarding and ongoing education for LPNs, RNs, MAs, and clinic teams related to standardized processes.
  • Provide hands-on coaching, technical assistance, and mentoring to clinic teams to support implementation and sustainability of PCMH and quality improvement initiatives.
  • Reinforce consistent expectations for nursing practice and team-based care delivery.
  • Assist in developing, implementing, and monitoring corrective action and performance improvement plans.
  • Utilize structured improvement methodologies (e.g., PDSA) to drive sustained change.
  • Work collaboratively with nursing leadership, medical providers, clinic managers, quality, and operations teams.
  • Promote a culture of accountability, continuous improvement, and patient-centered care.
  • Communicate clearly regarding expectations, progress, barriers, and outcomes related to PCMH and nursing practice improvement.
