Role Summary: The Director of Care Management provides strategic leadership and administrative oversight for the Care Management Department (CMD) including the MI Choice Medicaid Waiver Program, Community Transitions Program with potential for additional programs for older adults and persons with disabilities. This role ensures all program operations achieve contractual, accreditation, and agency goals while remaining in strict compliance with state and federal standards, including those set by the Michigan Department of Health and Human Services (MDHHS). Responsibilities and Duties Ensure operational policies and procedures, contractual and accreditation standards are met . Supervise and evaluate the MI Choice Manager, Vendor Contracts and Compliance Manager, and Community Transitions-Social Workers. Cultivate a high-performance collaborative culture by providing intentional mentorship and coaching to direct reports that encourages continuous learning, autonomy, and successful performance goal outcomes. Provide evaluation of program operations and services as measured by specified quality protocols and remediation plans . Build partnerships with other organizations to further the reach and impact of our programs. Participate in statewide and community meetings, events and conferences reporting relevant information back to the team. Collaborate with other departments within the agency to ensure that CMD activities are aligned with the TIC's mission and goals.
