About The Position

On- call position with NO guaranteed hours to support our individuals with all of their needs. Ensure their health & safety needs. Assist them with their career goals, be a mentor and example to all. Requirements Availability & Flexibility- respond within a required timeframe (30-60 minutes) Reliability- Consistent communication (phone, email, or messaging) Time Management- Ability to manage on-call shifts alongside regular duties Provide training, supervision, instruction and support to individuals in all agency settings, on site or in the community. Ensure that you are aware where your assigned individuals are during the day, let another staff know that you are leaving the floor and ensure that they have taken responsibility for your individuals until you get back. Attending agency required training and meetings and gives input on individual plan development. Assist to develop and track all worker goals and other daily documentation Be aware of all Mealtime Procedures and always supervise individuals during meals Provide transportation as needed for supported employment opportunities Provide on-going necessary support as indicated by their IP for each worker. Assist them to complete the tasks as independently as possible assisting if needed for the task to be completed properly Assist staff & individuals during fire drills or other emergency situations Assist in providing a positive program environment to staff & participants Complete and maintain all daily individual documentation and worker weekly notes. At the end of each month remove all data collection sheets and weekly notes from each person’s book that you are assigned that day. Ensure completion of all entries and turn into the Assistant Manager Ensure all extra documents are filled out by end of day if applicable, i.e.: 255, Seizure protocol, behavior data, ABC’s, minor incidents, etc. At the beginning of each month ensure that the new month’s data collection pages and weekly notes are put into the books that you are assigned that day Submit your COMPLETED timesheets and mileage forms to your manager by Friday PM Submit your Auto Insurance Declaration page to the Administrative Assistant upon renewal or every 6months Document appropriately in all Travel Books when using Company Vehicles It is your responsibility to let your immediate supervisor know if any of your trainings are getting ready to expire. Please give them 60 days’ notice of expiration Be on time & in the common area until the individuals start their shift Stay with your individuals at the end of the day until they are out of the building Be a team player Learn & follow all policy & procedures of the agency Learn and follow all safety & emergency procedures for the agency Be involved in OWC fundraising events Perform other duties as assigned, or as they arise. Assist individuals with toileting and hygiene issues as indicated in Level of Need No benefits package with this position Requirements QUALIFICATIONS FOR THE JOB: GED/High School diploma 3 years’ experience in a human service setting (preferred) Must be reliable, dependable, and flexible with your hours Must possess proven experience and success CPR/First Aid, Med Cert, PMT a plus Must possess valid driver’s license and have 5 years’ experience clean driving Must be able to work as part of a team as well as be independent KEY COMPETENCIES: Computer skills, Organizational skills, Interpersonal and Communication skills, Problem solving skills, good verbal and written skills, caring and nurturing, honest, reliable and punctual No breaks are given in this position as you are responsible for the safety and well-being of individuals PHYSICAL REQUIREMENTS: Must be able to lift & carry a minimum of 40 pounds

Requirements

Responsibilities
