Requirements
- High school diploma or equivalent required.
- Experience in direct support, disability services, or related areas.
- Driver's License (Required)
Responsibilities
- Provide person-centered support in a private home and the community
- Implement Individual Support Plans, behavior supports, and therapeutic routines
- Assist with life skills, household responsibilities, and community activities
- Coordinate medical appointments and support overall health and wellness
- Collaborate with families, caregivers, clinicians, and team members
- Document services and maintain confidential records in accordance with agency standards
