Description: Screen and prioritize acute care patients based on hospital policy and procedures including standards of care. Gather necessary data to complete assessment, intervention and plans monitoring. Document all finding in medical record. Includes but not limited to assessment/re -assessment, calorie count, education, follow up monitoring. Maintain productivity reports. Diagnose nutrition related problems based on data collected during assessment, including review of the medical record comprising past medical history, biochemistry and laboratory results, patient interview and collaborative finding from the interprofessional team. Develop intervention and plans for monitoring. Set measurable goals. Communicate plan of care with patient and interprofessional team. Document all finding in medical record. Accurately complete nutrition focused physical exam using appropriate tools, PPE and evidence-based guidelines. Develop plan of care based on findings. Document all finding in medical record such as malnutrition and micronutrient deficiencies. Communicate plan of care with interprofessional team. Manage enteral and parenteral nutrition support. Assess patients for proper indications. Develop intervention for nutrition support. Formulate plan of care and monitoring parameters using evidence-based guidelines for Nutrition Support Therapy. This includes PO supplements, enteral feedings and TPN/PPN. Document all finding in medical record. Communicate plan of care with interprofessional team. Participate in various initiatives to improve patient care by contributing to departmental/ interdepartmental performance improvement programs. Discuss outcomes with leadership. Assist with presentation of finding to team and leadership. Function as a resource for the team in their area of specialty/certification. Educate and train team members to enhance the patient experience and provide optimal nutrition plan of care. Support evidence-based strategies to improve patient outcomes. Complete within proper timeline. Act as a preceptor for dietetic interns. Act as a liaison to nursing/ patient care area staff. Participate in interdisciplinary care rounds and meeting. Cultivate relationships with interprofessional team. Communicate plan of care with interprofessional team as well as department. Actively participates in a hospital committee. Annually prepare required competency in-service for team and submit objectives to AND/CDR for continuing education credits. Ensure therapeutic diet compliance and patients’ satisfaction by collaboration with diet office/ kitchen teams. Verify and communicate with physician regarding diet orders. Enter diet orders, supplements, and enteral feedings correctly into computer system. Participate in staff meetings or review meeting minutes. Disseminate unit specific information to department. Communicate events in patient care areas and shares best practice with team.
