This is a critical role in coordinating food service activities and nutrition care process for our residents, to ensure their nutritional care needs are met! Requirements

  • High School diploma required.
  • At least two years of related experience, preferably in a hospital or nursing home, assisted living community adult family home or memory support unit.
  • Excellent interpersonal and communication skills and the ability to work with staff and external constituents
  • Able to provide dining services to elderly that meet or exceed hospitality and service standards
  • Ability to effectively communicate with Residents, family members, guests, vendors, other department heads, and regulatory agencies.

Responsibilities

  • Work with Dietary Manager to ensure all charts and medical records are up to date and followed by team members.
  • Monitors all diet changes to ensure compliance; Review resident diet information and discusses requests, changes, and inconsistencies with resident and staff.
  • Create Resident Care Plan upon admission and when changes in nutrition status occur.
  • Enter nutritionally relevant data on each resident as required by MDS standards and protocol.
  • Review dietary intake records, do visual meal rounds and monitor food intake documentation.
  • Assist Dietary Manager to ensure that all HAACP related forms and logs are filled out correctly and timely.
  • Assist in maintaining storage of records for survey review.
  • Inspects food and food preparation and storage areas as assigned by the Dietary Manager.
  • Perform other duties as assigned
