About The Position

DaVita is seeking a Licensed Practical Nurse (LPN) to provide life-giving care in a hospital setting to patients and their families dealing with end-stage renal failure or chronic kidney disease. The role emphasizes delivering empathetic and compassionate care. LPNs will be responsible for the complete ongoing care of each patient, ensuring their dialysis treatment stays on track, and will work in conjunction with dialysis RNs. The position requires working 3-4 days a week with 12+ hour shifts, with possible travel to cover other SNF clinics in the region and alternating Saturdays. The role is located at 5520 Broadview Rd Parma, OH. Patient care involves direct one-on-one interaction to provide safe, comfortable, and hygienic dialysis treatment. Dialysis care is a complex but fulfilling service line, requiring meeting or exceeding clinical outcomes and multitasking to meet the needs of the service, patients, and SNF partners. Training includes a combination of classroom and hands-on learning through DaVita’s award-winning programs, which may take place in a facility or training clinic. Support for other local SNF dens may also be required.

Requirements

  • Current LPN-LVN license in the state of practice required
  • Current CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency in computer systems of DaVita within 90 days of hire required

  • Minimum of one year's experience in a hospital or clinical setting preferred

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track
  • Work in conjunction with dialysis RNs to provide dialysis care
  • Provide safe, comfortable and hygienic dialysis treatment through direct one-on-one patient care
  • Meet and/or exceed clinical outcomes
  • Multitask to meet the needs of the service, the patients and SNF partners
  • Support other local SNF dens (as requested)
