About The Position

DaVita is seeking a Licensed Practical Nurse (LPN) to provide life-giving care in a hospital setting to patients and their families dealing with end-stage renal failure or chronic kidney disease. The role involves taking responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. This position requires working 3-4 days a week with 12+ hour shifts, with possible travel to cover other SNF clinics in the region and alternating Saturdays. The LPN will spend the majority of the day in direct one-on-one patient care, providing safe, comfortable, and hygienic dialysis treatment. DaVita emphasizes meeting and/or exceeding clinical outcomes and expects multitasking to meet the needs of the service, patients, and SNF partners. Training includes a combination of classroom and hands-on learning through DaVita’s award-winning programs, which may take place in a facility or a training clinic. Requirements

  • Current LPN-LVN license in the state of practice
  • Current CPR certification
  • IV certification if required by state regulations
  • High school diploma or equivalent
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system
  • Basic computer skills and proficiency in MS Word and Outlook
  • Functional proficiency in computer systems of DaVita within 90 days of hire

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track
  • Work in conjunction with dialysis RNs to provide dialysis care

  • Multitask to meet the needs of the service, the patients and our SNF partners
  • Support other local SNF dens as needed
