About The Position

The diabetic educator will provide inpatient, outpatient, employee, and community education. The diabetic educator will work closely with patients and families to improve overall knowledge of diabetes and how to modify risk factors of the disease. The diabetic educator will meet patients where they are in their disease progression. He/she will draw upon a variety of tools and resources to assist patients in improving quality of life and disease maintenance including, but not limited to, dietary restrictions and recommendations, medication adherence, and glucose monitoring.

Requirements

  • Either a Registered Dietitian (RD) or Registered Nurse (RN), licensed in the State of Illinois
  • For RD, minimum of Bachelor of Science in Food and Nutrition
  • For RN, minimum of Bachelor of Science in Nursing
  • Minimum of one year of experience in providing diabetic education required
  • Minimum of three years of hospital experience
  • Current BLS certification required

Nice To Haves

  • Certification Board for Diabetes Care and Education (CDCES) preferred or obtained within one year of hire

Responsibilities

  • Document care plans and other nutritional information to be communicated to physicians or providers. Makes diet recommendations to physicians or providers as needed.
  • Document all patient care and education provided in accordance with hospital policies and regulatory requirements.
  • Identifies patients at risk for or are diagnosed with diabetes. Provides proper education when ordered by the physician or provider. Assess patients' knowledge and understanding of diabetes and develop personalized education plans to meet their needs.
  • Transitions of inpatient diabetics to outpatient education when requested by physician or provider.
  • Maintains all required and mandatory requirements of the hospital and its governing bodies (i.e. DNV, IDPH, CMS, State, and OSHA). Ensure compliance with ADA Education Recognition requirements and National Standards of Diabetes Care.
  • Serves as Coordinator for Diabetes Self-Management Education Program (DSME).
  • Schedules and conducts individual and group diabetes education sessions for patients and their families, focusing on diabetes management, nutrition, medication understanding, and lifestyle modifications.
  • Involved in curriculum development, program goals, outcomes, and performance improvement for the DSME program.
  • Participate in community outreach programs to promote diabetes awareness and prevention.
  • Maintains certification and continuing education requirements.
  • Provides DSME program information to community physicians and providers.
  • Coordinates programming for Diabetes Support Group (when active) and assists with Diabetes Newsletter preparation.
  • Conducts educational clinics for hospital personnel when requested.
  • Maintain required training and/or education per hospital guidelines.
  • Follow and maintain all lawful safety, health requirements and regulations, and follow protocols for required protective equipment for peers, patients and community.
  • Promptly report safety issues such hazardous conditions, job-related injury or illness, and seek guidance or treatment.
  • Adheres to all professional standards, policies and procedures, federal/state/local requirements, and regulatory organizations.
  • Maintains strict confidentiality of patients and records.
  • Portrays a professional and positive attitude for peers, patients, and community.
  • Assumes other duties as assigned.
