About The Position

JOB SUMMARY: Diabetic Educator is responsible for providing diabetes education and management skills to patients and families in one-to-one and group settings. This includes assessment, goal setting, education, phone follow-up and evaluation of patient learning on a continuing case management basis in collaboration with the entire health care team. PRIMARY JOB DUTIES: Adheres to Brodstone Healthcare’s Mission, Vision and Values. Adheres to Brodstone Healthcare’s Standards of Behavior. Assesses patient and family knowledge and skill, establishes mutual behavioral goals, formulates an education plan and provides a means to accomplish these goals and documents actions and response. Implements patient and family education in all aspects of diabetes care, according to the best practice for both the inpatient and outpatient settings, and documents response. Assesses patient knowledge and adherence to diet and refers patient and family to the dietitian when appropriate. Collaborates with members of the interdisciplinary health team and referring providers to define and achieve patient/family goals, updating team members as appropriate on patient progress. Serves as a gate-keeper of services to the patient, facilitating accomplishments of outcomes of care as determined by the health care practitioners. Is responsible for coordinating services in an effort to assure quality, improve continuity of care, and keep costs reasonable. Assesses how well the family is coping and intervenes when necessary. Participates in Brodstone Community Wellness initiatives including but not limited to health fairs, public seminars, educational programs. Assists in the attainment of ADA recognition of the Diabetes Center’s Diabetes Education Program. Assists in ongoing assessment of knowledge and skills in diabetic education of the staff. Provides the staff with diabetic care-related literature that is relevant and current. Communicates in a professional manner with ancillary departments, providers, allied health professionals, visitors and community resources. Promptly reports pertinent data concerning patients to the referring providers. Actively participates on assigned committees and reports pertinent data to the staff. Consistently supports the Compliance Program and participates in on going monitoring of activities with respect to compliance related issues. Maintains health records in compliance with policies of the organization. Shall have the ability to work with others in a team environment, the ability to accept direction from supervisors, the ability to follow work rules and procedures, and the ability to accept constructive criticism. As part of your job function you will need to access PHI information for treatment, hospital operations or payment purposes. See hospital/department policy and procedure for access granted for this job function. This employee may be required to have a valid driver’s license to drive the hospital vehicle. Abides by the policies and procedures of the facility. Standards of conduct in the Compliance Plan and participates in ongoing monitoring of activities with respect to compliance. Attends and participates in departmental meetings / in-services, Brodstone Healthcare quality improvement program, etc. Performs other duties as assigned.

Requirements

  • Associates Degree
  • State of Nebraska RN License or Multi-State RN License
  • BLS certification within 6 months of employment
  • ability to work with others in a team environment

  • 1-2 years of Diabetic Education work experience or experience with Chronic Disease management.
