About The Position

The Diabetes Program Manager is responsible for overseeing the day-to day operations of Diabetes Self-Management Education Services (“DSMES”). The Diabetes Program Manager ensures that the National Standards (“NSDMES”) are met and maintained at all times and is responsible for facilitating DSMES services for patients as a member of the professional DSMES team, in addition to providing general nutrition intervention/counseling to patients for continuity of care.

Requirements

  • Registered Dietician (“RD”) /Certified Dietician Nutritionist (“CDN”) - New York State licensure
  • Minimum of Bachelor of Science in Nutrition plus
  • One (1) to two (2) years nutritional experience in a clinical/community health setting
  • Maintain current Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (“BC-ADM”) certification OR obtain a minimum of 15 hrs of diabetes-related CEU’s credits provided by NCBDE approved CE providers within 12 months prior to accreditation and annually throughout the accreditation 4-year cycle.

Responsibilities

  • Ensures the implementation of the National Standards and oversee the DSMES services.
  • Responsible for all components of DSMES, including the implementation of the standards, ensuring services are evidence-based, making sure service design incorporates population needs, ensuring ongoing service evaluation and continuous quality improvement plan is reviewed at least annually.
  • Collects and evaluates data to identify gaps in DSMES.
  • Provides feedback on the performance of the DSMES services to team members, referring practitioners, and leadership team.
  • Utilizes data mining to inform payers and members of the health care team of the clinical outcomes of DSMES.
  • Coordinates a DSMES Advisory Group, seeking input from key stakeholders on services, quality, utilization, access, and sustainability.
  • Monitors and facilitates the maintenance of DSMES team member qualifications (CE credits, training, competency, licensures, and registration).
  • Responsible for maintaining Association of Diabetes Care and Education Specialists (“AADE”)/American Diabetes Association (“ADA”) recognition and participation in the evaluation of the DMES services effectiveness.
  • Organizes and develops nutritional care plans and individualized therapeutic diets as needed.
  • Completes necessary reports and forms and documents patient services in the appropriate medical record.
  • Facilitates related services for health center clients as appropriate with respect to their confidentiality and privacy.
  • Organizes and distributes nutrition education materials.
  • Conferences with the Director of Nutrition as necessary to seek consultation and share information that is pertinent to patient care.
  • Plans, conducts, and participates in nutrition education presentations, workshops, and health fairs for the agency.
  • Attends conferences and participates in trainings/informal sessions as requested by Director of Nutrition.
  • Provides coverage for nutritionists at centers as assigned.
  • Performs other related duties as assigned.
