About The Position

SUMMARY: The position provides Diabetes Nutrition Education to the Omaha Tribe population in concert with the Omaha Diabetes Program and the Omaha Wellness Center. The position also carries Nutrition Program responsibilities separate from the Omaha Diabetes Program. The Diabetes Nutritionist functions in clinical and community settings. ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following: Serves as Dietitian and Nutrition Educator, providing culturally appropriate direct patient care to individuals and groups in homes, community and institutional settings. All efforts are directed toward achieving control in newly diagnosed Diabetics, chronic Diabetic clients and Gestational Diabetics and their families. Plans, develops, and implements and evaluates the Nutritional Education component of the Diabetes Program, coordinating with team members with whom program goals and objectives are developed. Conducts Nutritional Assessment of clients, developing Nutritional Care Plans with them and their families consistent with cultural, social, and physiological requirements. This is accomplished through counseling, medical care, video conferencing, and chart review. Reviews, selects or prepares suitable education and counseling materials on normal and therapeutic nutrition for use by professional and paraprofessional staff, provides services to clients and community. Plan for and participate in group teaching and training of clients, staff, and community members to expand services and increase public awareness of Diabetes Control. Provide consultation to professional and paraprofessional staff who provide services and encourage continuity of care. Records information in Medical Records (progress notes) regarding nutritional requirements, patent progress, education, follow-up and similar matters. Uses and contributes to Diabetes Registries, tracking systems and referral services to help provide comprehensive Diabetes care. Evaluates the effectiveness of Nutrition Program of the Omaha Diabetes Program. Evaluates the effectiveness of the Nutrition Program of the Omaha Wound Care Program. Coordinates the diabetes Program Nutrition Program with the Tribal Programs (i.e. WIC, Headstart, and Elderly feeding Program) and with potential federal and state sources of Nutritional Support. Establishes Nutritional Policies and guidelines for the Diabetes Program/Wellness Center staff in addition to conducting community based education. Assist in developing plans and participates in community based prevention and control programs emanating from Omaha Wellness Center. Perform other duties as assigned

Requirements

  • Must possess a current Registered Dietitian certification or professional registration as a Registered dietician.
  • Must have a valid driver's license and be insurable.
  • Ability to adapt consultation, training or nutrition care plans to suit the individual patient or family.
  • Takes part in Diabetes support group activities.
  • Ability to prepare acceptable materials for use by communication media.
  • Ability to present ideas clearly and concisely, orally and in writing.
  • Ability to plan and organize work effectively and independently.
  • Must be willing to participate in cultural sensitivity training to gain valuable insight about the Omaha culture.
  • Must pass background check, physical, and pre-employment drug screen.
