About The Position

The Diabetes Educator is responsible for providing education programs/services to all participants of the Capital Region Diabetes and Endocrine Center. The Diabetes Educator is utilized as a resource consultant to staff members, physicians and other health professionals as they provide diabetes education within the agency and in the community. Leadership is evidenced through collaborative program design, implementation, and evaluation; participation in departmental and interdepartmental committees, through clinical consultative services; and commitment to enhancing personal professional growth. Hospital philosophy/goals and American Diabetes Association Standards of Care guide these actions EDUCATION AND EXPERIENCE REQUIREMENTS: Registered nurse with current license to practice in New York State. At least two years experience in diabetes education. A B.S. in Nursing preferred. Certified Diabetes Educator required or a specific plan to achieve certification is evident. Dietitian with a BS or MS in Nutrition.Must be registered by the American Dietetic Association and NYS Certified. At least 2 years experience in diabetes education and Certified Diabetes Educator required. PHYSICAL REQUIREMENTS: The position is located indoor and some outdoor work under normal working conditions. Should be able to push/pull 30 pounds, lift/move 20 pounds from floor to table, and be able to perform moderately difficult manual manipulations requiring eye-hand coordination such as patient care and use of computers and complex equipment. Mobility requirements may include the ability to sit and stand for prolonged periods of time in addition to being able to walk for a reasonable length of time and distance. Valid NYS driver’s license and access to a car for travel to perform education activities at multiple sites. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language. RESPONSIBILITIES OF THE POSITION The Diabetes Educator functions as an expert clinician in the assessment, planning, implementation, and evaluation of care for the patient with diabetes across the lifespan (pediatric, adolescent, adult, and geriatric.) The Diabetes Educator designs, implements and evaluates diabetes education programs of varying complexity for professional staff, people with diabetes, and those at risk for developing diabetes The Diabetes Educator acts as a consultant in the community and to all members of the health care team involved in the care of the patient with diabetes Assumes a leadership role in implementing innovations in clinical practice to promote continuous quality improvement and desired patient outcomes for the patient with diabetes Adheres to the policies and procedures as outlined in the Exposure Control Manual regarding Universal Precautions Bloodborne Pathogen Disposal of Needles Syringes and Sharps Cleanup Procedures for blood and body fluid spills Storage, Handling and Disposal of Medical Waste Adheres to hospital policies and procedures related to: Mandatory education Annual health assessment Licensure verification Ellis Medicine is committed to creating a diverse environment and is proud to be an equal opportunity employer. Salary Range: $25.72-$38.57 /hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Requirements
