This program supports individuals across all stages of diabetes management, including type 1, type 2, prediabetes, and gestational diabetes. The multidisciplinary team, located in a state-of-the-art building at 10 Members Way in Dover, NH, consists of board-certified endocrinologists, certified diabetes educators, registered dietitians, and specialized nursing staff who collaborate to deliver personalized care plans. Services include individualized and group education sessions focusing on blood glucose monitoring, nutritional counseling, exercise programs, and medication management. Additionally, the hospital conducts community outreach through educational classes, support groups, and seasonal camps for children with diabetes. The program emphasizes a patient-centered approach, aiming to empower individuals with the knowledge and tools necessary for effective self-management and improved quality of life. The organization is seeking a part-time, 30-hour Diabetes Educator to join its Diabetes Services team. The weekly schedule for this role will be Monday, Tuesday, and Thursday from 7:45 a.m. to 5:45 p.m. This position is responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. It involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs. The Diabetes Educator will play a key role in empowering patients through education, clinical guidance, and ongoing support, working closely with providers and caregivers to improve long-term health outcomes.
