How You’ll Help Transform Healthcare: Hours: Day shift, occasional evenings Plans and provides individual and group instruction to meet requirements established in the National Standards for Diabetes Self-Management Education Programs and American Diabetes Association for participants in the Diabetes Self-Management Program. Provides advanced and specialized knowledge and communicates recommendations for changes in patient treatment to physicians and hospital staff; documents recommendations and monitors changes and outcomes; facilitates the development and implementation policies, procedures and protocols related to diabetes management; evaluates and tracks data for outcomes and related performance improvement measures; develops and oversees educational opportunities and teaching resources for hospital staff. Serves as resource and mentor to non-certified diabetes educators. Age specific assessments are completed and used to develop an educational plan of care. Collaborates with other educators and program coordinator to develop and revise program educational materials to meet the needs of the target population and ADA content requirements. Collaborates with program participants to identify individualized educational needs. Facilitates and collaborates with other educators and program instructors in the development of a written curriculum that includes methods and materials, and the means for evaluating achievement of the objectives for each content area of the program session. Reviews and interprets blood glucose reports and relevant clinical data to identify inpatients for consult and establishes appropriate plan of care Coordinates group and individual instructional activities Cooperates with management and staff in meeting the goals and objectives of the program. Serves as a diabetes educator primarily for groups and individual: provides presentations for the public, civic organizations, business and industry, health care employee groups and the community at large. Provides client follow-up and case management, referring to the Case Management Department as appropriate. Prepares and submits documentation of operational statistics and outcomes related to programs and activities. Develops and executes performance improvement and quality assurance outcomes projects with program manager Applies knowledge of growth and development to appropriately educate patients Assists the Program Manager in coordinating activities of resource personnel. Actively participates in multidisciplinary team meetings established for diabetes programs Demonstrates accountability through continuous professional growth, service excellence and support of Carilions mission.
