About The Position

The incumbent will develop, manage, and coordinate all aspects of the outpatient diabetes program and is responsible for outreach to physician practices as a referral source for the diabetes program.

Requirements

  • Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education
  • One (1) year experience in health and fitness
  • Computer skills, particularly Excel and Word.
  • Ability to understand written and oral communication.
  • Ability to lift 25 pounds.
  • Ability to push/pull 100lbs.
  • Ability to stand for several hours per day.

Nice To Haves

  • ASCM, Clinical Exercise Physiology Certified

Responsibilities

  • Identifies education needs of the community and those with diabetes.
  • Attends diabetes advisory committee meetings as assigned.
  • Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.
  • Assists in maintaining the American Diabetes Association certification as applicable.
  • Participates in community outreach and educational support.
  • Advocates on behalf of the patient and families.
  • Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
  • Participates in performance improvement activities to assure quality care.
  • Provides administrative support and coordinates the management and operations of the Diabetic Institute
  • Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.
  • Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
