About The Position
The Diabetes Care # Education Specialist RN (DCES)#provides expert inpatient and outpatient education for individuals with diabetes in alignment with Parkview Health Standards of Care#and American Diabetes Association (ADA)#accreditation guidelines. # This role integrates clinical expertise with self-management education to promote optimal health outcomes for patients with diabetes and related comorbidities. The DCES collaborates with patients to establish individualized goals, supports behavior change, and promotes lifelong self-management to improve quality of life.
Requirements
- Associate of Science in Nursing (ASN)## Required.
- Bachelor of Science in Nursing (BSN)## Required within 3 years of start date.
- Current RN License## State Board of Nursing.
- Current CPR certification## Required.
- Minimum 5 years of RN experience#in a clinical setting # Required.
- Strong verbal, written, and interpersonal communication skills.
- Demonstrates excellent customer service and ability to prioritize and work independently.
- Proficient in applying evidence-based practice and clinical judgment.
Nice To Haves
- Certified Diabetes Care and Education Specialist (CDCES)#or Board Certified in Advanced Diabetes Management (BC-ADM)## Preferred.
- Minimum 5 years of RN experience#in a clinical setting # Preferred.
Responsibilities
- Delivering evidence-based diabetes education and clinical care.
- Reviewing medications for therapeutic effectiveness.
- Collaborating with interprofessional teams to develop and evaluate patient-centered care plans.
- Documenting all patient interactions accurately and promptly in the EMR.
