About The Position

The Departure Lounge Coordinator will communicate discharge recommendations of the clinical team to patients, caregivers, and/or the interdisciplinary team. This role is located at the St. Vincent facility in Indianapolis, IN, within the Departure Lounge department, and operates on a full-time day schedule. Requirements

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Responsibilities

• Participate in care planning and communicate information between patient and team.
• Document plan in medical record.
• Gather and arrange the necessary materials to enable patient discharge.
• Order supplies/equipment for patient specific needs as identified by clinical team.
• Perform administrative and clerical duties in support of unit/department.
