The Dementia Support or GUIDE (Guiding an Improved Dementia Experience) Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. It will achieve these goals through Medicare payments for a comprehensive package of care coordination and care management, caregiver education and support, and respite services. Dementia affected more than 6.7 million Americas in 2023, with 14 million projected cases by 2060. The Dementia Support Navigator works closely with an integrated team to implement quality dementia care initiatives and support the operational success of the GUIDE Model, known as Dementia Support. Additionally, this role facilitates collaboration between caregivers, primary care physicians, and specialists to enhance continuity of care. The navigator plays a key role in community education efforts by offering training sessions, workshops, and informational resources to improve dementia awareness and caregiver preparedness. By fostering strong partnerships with healthcare providers, social service agencies, and community organizations, the Dementia Support Navigator ensures comprehensive support for individuals living with dementia and their families.
