About The Position

The Relief position will give you an opportunity to gain experience and make a difference in someone’s life while working with people with Mental Health challenges and Developmental Disabilities. Open Sky's Day Habilitation programs serves adults between the ages of 23-85 with a primary diagnosis of intellectual disabilities, though some individuals may also have mental health diagnoses and/or physical challenges. Most participants require supportive structures and staff assistance to maximize their involvement in activities within the program as well as within the local community. Our staff educate through the creation and implementation of individualized service plans, daily curriculum/activities, and have a heavy focus on community integration. As a Per Diem Day Hab Support Staff you will provide support to the individuals served in a direct care role by ensuring a supportive and structured environment. You will also provide guidance, advocacy, and life skills education for individuals served within the Day Program. The organization recognizes skills, location, training and population served in certain programs by offering an additional hourly differential. Requirements

• High School Diploma, GED or equivelent, required.
• Valid Drivers License and acceptable driving record, required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans.
• Assist individuals to access and/or provide transportation.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Review and complete all required documentation.
