JOB SUMMARY: To train and assist individuals with developmental disabilities in self-help, socialization, personal care, and adaptive skills. Leads group activities, as directed by the Day Services Manager, by performing the following duties.

Responsibilities

  • Provides participant specific assistance and skills training for personal care skills, activities of daily living, first aid/safety procedures and supervision of self-administration of medication in accordance with agency policies and procedures (examples: feeding, toileting, lifting and transferring).
  • Provides leadership to co-workers to adhere to the daily activity schedule, promote positive day center outcomes and mission of the agency.
  • Provides leadership to co-workers to maintain work area – cleaning tables, chairs, and supplies daily/weekly as scheduled. Assures supplies are kept organized and in working order. Return supplies to designated area daily.
  • Assists participants with the acquisition, retention, or improvement of self-help, socialization, and adaptive skills training.
  • Implements care plan to achieve specific outcomes derived from the participants' preferences, needs and interests.
  • Implements behavioral support plans of participants to reduce inappropriate and maladaptive behaviors and assist with acquiring appropriate adaptive skills and behaviors. Implements approved crisis prevention, intervention and resolution techniques as needed.
  • Implement an array of meaningful, person-centered activities and opportunities of fine arts, recreation, leisure and socialization throughout the day as indicated by the daily schedule, lesson plans, care plans and the individual’s goals.
  • Ensures the activities and opportunities promotes: 1. Dignity and respect 2. Health, safety and well-being 3. Individual choice, control and responsibility 4. Personal growth and accomplishments
  • Provides active support, direct assistance and supervision to the individuals while participating in day activities.
  • Demonstrates effective communication strategies and skills necessary to establish a collaborative relationship with participants.
  • Assists in the assessment and plan of care process to determine the needs, preferences, interests, and capabilities of each participant. Assists in reviewing the assessment process for inconsistencies and makes corrections as necessary.
  • Provides transportation as needed for participants.
  • Complete all required documentation such as (progress notes, goal tracking sheets, seizure tracking, positing tracking, communication logs, activity calendar, attendance logs, etc.) daily. Ensures all written and electronic documentation is accurate and submitted on time.
  • Compiles with policies and procedures to ensure confidentiality of individual’s information both written and spoken.
  • Maintain a positive and productive environment for participants, respecting individual’s rights, and reporting suspected abuse, neglect, and exploitation of any individual.
  • Provide required documents for personnel file compliance; adhere to training requirements and re-certifications as requested. Attends all assigned departmental and program specific Development Meetings.
  • Obtains annual physical / TB test and ensures that results are signed by appropriate medical staff.
  • Develop and maintain positive and productive relationship with participants, co- workers, guardians, family and community members.
  • Support program participants in their personal growth and development, respecting cultural, ethnic, spiritual and individual differences.
  • Maintain objectivity in position in order to set appropriate limits while working with participants.
  • Adhere to all agency policies and procedures.
  • Other duties as assigned.
