About The Position

The Critical Time Intervention (CTI) model is a time-limited, evidence-based care management approach focused on ensuring continuity of care for individuals with serious mental illness during critical transitions from inpatient settings back into the community. This position is part of a unique collaborative project between Unity House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County. The CTI Care Manager will work as part of a multidisciplinary team to provide comprehensive, person-centered support to participants during pivotal moments in their lives. The Care Manager will engage participants in individualized care planning, connect them to community resources, support skill development, provide crisis case management, and assist with building sustainable support systems.

Requirements

  • Associate’s Degree required; Bachelor’s Degree preferred
  • Experience working with individuals living with mental illness, substance use disorders, or other disabilities preferred
  • Proficiency in Microsoft Office and web-based systems
  • Prior experience with the Medicaid delivery system preferred but not required
  • Valid, clean New York State driver’s license required
  • Reliable transportation required
  • Ability to work effectively in a fast-paced environment

Nice To Haves

  • Experience working with individuals living with mental illness, substance use disorders, or other disabilities preferred
  • Prior experience with the Medicaid delivery system preferred but not required

Responsibilities

  • Provide care management services to CTI participants across both Rensselaer and Schenectady Counties
  • Complete person-centered care plans in collaboration with participants
  • Develop and coordinate linkages to community-based resources
  • Assist individuals in navigating complex service systems
  • Provide a range of treatment, rehabilitation, and supportive services
  • Develop and maintain strong relationships with community partners
  • Complete required documentation and administrative tasks accurately and in a timely manner
  • Promote participant wellness, recovery, and self-management
  • Foster a culture of inclusion and belonging
