About The Position

Established in 1980, Greater Lawrence Family Health Center, Inc. (GLFHC) is a mission-driven, multi-site, non-profit organization serving the diverse communities of the Merrimack Valley. With over 700 employees, GLFHC is nationally recognized for excellence in community medicine and provides comprehensive care in family practice, pediatrics, internal medicine, geriatrics, and supportive services. GLFHC is seeking a CSS Social Services Coordinator to support the Community Support Services (CSS) Department. This role is responsible for coordinating referrals, pre-visit planning, no-show monitoring, and performance reporting to improve patient access, service delivery, and program effectiveness. The Coordinator plays a key role in communication, data tracking, and operational support across departments.

Requirements

  • Minimum 2 years in healthcare, community services, or administrative support preferred
  • Strong organizational and data management skills
  • Proficiency in Microsoft Excel, Word, and Outlook
  • Experience with EHR systems preferred
  • Strong verbal and written communication skills
  • Valid Massachusetts Driver’s License and reliable transportation
  • Degree in Human Services, Public Health, Healthcare Administration, or related field preferred
  • Combination of education and experience considered

Nice To Haves

  • Bilingual/Bicultural (Spanish/English) strongly preferred
  • Medical Interpreter Certification (or completion within 12 months)
  • Community Health Worker Certificate (or completion within 12–18 months)

Responsibilities

  • Referral Coordination: Receive, log, and track referrals to CSS Communicate with referring providers, patients, and staff Prepare referral status reports for meetings
  • Pre-Visit Support: Coordinate pre-visit planning and required documentation Track completion of pre-visit requirements Address missing information as needed
  • No-Show Monitoring & Reporting: Track patient attendance for CSS services Prepare no-show reports and identify trends Support strategies to reduce missed appointments
  • Performance Monitoring & Data Reporting: Collect and analyze referral, attendance, and outcome data Prepare monthly, quarterly, and ad-hoc reports Support quality improvement and compliance efforts
  • New Hire Training & Preparation: Prepare onboarding materials and system access Support training on workflows and procedures Provide guidance during orientation
  • Administrative & Program Support: Assist with scheduling, meetings, and documentation Maintain accurate records in EHR and program databases Support communication across CSS and other departments
