Requirements

  • As required by contract
  • Current, or eligible for, WI and/or IL RN license
  • Graduate of an accredited CRNA program
  • Current, or eligible for, WI and/or IL APNP license
  • DEA, or eligible for, licensure
  • Current CPR, ACLS and PALS certifications
  • Ability to pass the Wisconsin Caregiver Background Check and/or Illinois Health Care Worker's Background Check
  • Per Unit Requirements: NALS
