This position is responsible for directing the daily activities, meeting the daily needs, and providing independent living skills education to clients. This position directly assists clients with emotional, physical, spiritual, social needs and assigned administrative tasks to promote independent living abilities. This position operates under minimal supervision and must exercise tact, judgment, and confidentiality in contact with the public, clients, and families, as well as fellow associates of the Center. Work is very broad in scope and requires a high level of professionalism.

Requirements

  • Education: At minimum, a High School graduate or GED.

Nice To Haves

  • A degree or further education in healthcare or healthcare management preferred but not required.
  • Experience: Previous experience in a mental health setting or human services field preferred but not required.
  • Previous experience as a nurse's aid preferred but not required.

Responsibilities

  • Assist in the admission process, including opening and registering new clients for MCT and CRSS by obtaining necessary data, taking, and recording vital signs, explaining facility policies and procedures, orienting clients to the CRSS and providing introduction to other staff and peers.
  • Assist the treatment team by identifying psychiatric symptoms, stressors, past psychiatric history, and other pertinent information to help form an appropriate clinical impression. This would include completing various assessments, based on education level and training, of the client’s ability to perform activities of daily living (ADL) and independent living skills.
  • Complete brief risk assessments and comprehensive risk assessments.
  • Complete other assessments that clients may need, such as PHQ9, GAD 7, HRSN, LOCUS, and safety plans.
  • Assist in the transportation, serving, and clean-up of all meals.
  • Responsible for all cleaning of the CRSS
  • Assess for abuse and/or neglect.
  • Utilize treatment plan goals and objectives, which accurately reflect the client’s needs and ability to meet discharge criteria. Goals and objectives are time limited and behaviorally stated.
  • Document timely (before the end of each shift) and accurately all relevant client information in accordance with Center policies and the treatment plan utilizing current systems and practices.
  • Share all relevant information with appropriate personnel, either immediately or at change of shift, applicable to the progress of the client.
  • Provide observation at level ordered by psychiatrist or designee.
  • Provide instructive activity/coping skill groups.
  • Provide supportive communication to clients such as listening to client problems, making observations, and attempting to establish and maintain rapport with clients, adapting communication style based on each individual client’s cultural, education and pathological needs.
  • Participate in team meetings providing input on assigned clients, when applicable.
  • Make rounds and conduct room, facility, and body searches as appropriate for facility as scheduled and necessary, documenting accordingly.
  • Complete 15 min checks on clients along with other precaution checks that are ordered and ensure it is documented appropriately.
  • Ensure a safe and therapeutic environment for all clients and CRSS staff.
  • Follow all assignments as directed by the Zones Sheet, Charge Nurse, and DON
  • Supervise completion of personal hygiene and task assignments, assisting as necessary, documenting accordingly.
  • Maintain awareness of potential safety hazards and report to charge nurse or supervisor timely.
  • Respond to recognized facility “codes” as necessary and appropriate.
  • Participate in initial and continuing training for seclusion and restraints and demonstrate proficiency.
  • Provide transportation for clients as directed/needed once all transportation options have been exhausted.
  • Educate and assist clients with independent living skills in coordination with their individual treatment plan.
  • Work with clients to help find resources prior to discharge and ensure all follow-up care is scheduled prior to discharge.
  • Work with clients one-on-one and in group settings designed to provide social support, presenting topics and situations pertinent to establishing and/or re-enforcing treatment goals. Directing and planning group activities.
  • Provide situation and verbal de-escalation with combative consumers as necessary per Center policies.
  • Must be eligible and meet all requirements for 4C van driving and remain in compliance with van driving policies throughout employment. Transportation in personal vehicles is not permissible.
  • Participate in on-call rotation as outlined by the department/location.
  • Follow Direct Client Care activity goals as dictated by department/location.
  • Maintain positive working relations and rapport with the communities served.
