The Crisis Registered Nurse (RN) is a core member of a high-fidelity Assertive Community Treatment (ACT) team within a Certified Community Behavioral Health Clinic (CCBHC), serving adults with serious mental illness receiving voluntary ACT services. The RN provides community-based nursing services, manages medication and health components of ACT treatment plans, and supports integrated behavioral and physical health care. Responsibilities include nursing assessment, administration and monitoring of psychiatric medications (including long-acting injectables as needed), crisis response, and care coordination. 1. Patient Care, Medications & Monitoring Assess clients’ physical and mental health needs. Administer and monitor medications, including injections. Watch for side effects and report concerns to the care team. Educate clients on medications and support adherence. Manage prescriptions, refills, lab work, and vital signs. Supervise LPNs and provide clinical guidance. 2. Teamwork, Crisis Response & Care Coordination Participate in daily ACT team meetings and care planning. Respond to mental health crises and assess medical stability. Coordinate care with doctors, hospitals, and community services. Support clients during hospital visits and transitions. Connect clients to resources like housing, medical care, and substance use treatment. Help develop crisis and safety plans. 3. Documentation, Compliance & Professional Duties Document all care and services accurately in the system. Follow healthcare regulations and program standards. Participate in audits, quality improvement, and required trainings. Maintain professional relationships with clients and partners. Take part in on-call coverage and support 24/7 services. Attend meetings and complete other assigned duties.
