About The Position

The Crisis Discharge Coordinator supports the Crisis Triage Center (CTC) multidisciplinary team by providing and tracking discharge coordination and planning for services that are complex, multi-system, and specific for the individuals at CTC. Through utilizing internal and external resources, the Discharge Coordinator assists the client with referrals and resources to contribute to continued mental health stabilization post-discharge from CTC.

Requirements

  • EDUCATION: High School Diploma or GED
  • EXPERIENCE: Entry level (minimum 1-year of job-related experience). Valid Washington Food Worker card.
  • This position requires driving clients in a personal vehicle on behalf of the agency. Therefore, the incumbent must meet all agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.
  • LICENSURE: Eligibility for Agency Affiliated Counselor Registration.
  • Maintain a working knowledge of current trends in community mental health, including health care reform and automation of job tasks using computer technology.
  • Maintain a working knowledge of mental health rehabilitation standards.
  • Excellent interpersonal and verbal skills.
  • Communication - Communicates clearly and concisely.
  • Computer skills - The ability to operate a personal computer, fax machine, printer, and copier proficiently.
  • Ability to deescalate potentially volatile situations with clients.
  • Ability to react quickly and perceive environmental stimuli accurately.
  • Ability to assist in building positive working relationships with staff of all agency departments

Nice To Haves

  • EDUCATION: A Bachelor’s degree in Psychology, Social Work or a Behavioral Health Field.
  • EXPERIENCE: Experienced (minimum 2 years of job-related experience). Relevant experience working with clients with severe and persistent mental illness.

Responsibilities

  • Coordinate and facilitate timely implementation of discharge plans for clients being supported at CTC who are experiencing a behavioral health crisis.
  • With the support of the Mental Health Professionals (MHP) and Behavioral Health Counselors (BHC), develop and coordinate discharge plans. that are: complex, multi systemic, and specific to clients in crisis who might be experiencing barriers to accessing resources and basic needs.
  • Under the direction of the MHPs and BHCs, support with completing case management tasks associated with discharge including but not limited to coordination of ongoing mental health, psychiatric, and/or substance use services, housing referrals, transportation supports, DSHS-related services, connection to outpatient medical and/or dental care, and any needed follow-up appointments for ongoing care.
  • Participate in daily staff organizational meetings and stabilization planning review meetings.
  • Facilitate approved group activities related to knowledge of resources and supports in the community and how to access after discharge.
  • Provide transportation for clients to and from appointments or resources in the community which can include DSHS, medical or dental care, court, etc.
  • Provide practical help to support clients with accessing any other necessary services to lend to mental health recovery following crisis stabilization.
  • Maintain effective communication and collaboration with CTC unit staff, any identified care teams and supports, and any additional parties related to discharge planning.
  • Complete all electronic and paper documentation involved in providing direct client care and maintain clinical information of individuals served by the unit in a confidential manner as outlined in the appropriate KMHS guidelines, manuals, policies and procedures.
  • In the absence or shortage of Behavioral Health Technicians (BHT), the Discharge Coordinator can also support with tasks related to the role of BHT which can include: Conduct safety checks of clients and the unit environment Support with the process of intake and discharge to include any paperwork or documentation involved Take referrals appropriately Prepare and serve meals
