How you make a difference The Admission Discharge Coordinator coordinates and implements discharge planning services including identifying local resources and coordinating appointments with programs and/or providers as needed. Discharge Planners may also provide individual and/or group psychoeducational sessions for individuals who are scheduled for release into the community.

Requirements

Education Graduate from an accredited school of nursing.
Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.

Nice To Haves

Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred.

Responsibilities

Provide discharge planning services, including MAT, medical, and mental health programs, in accordance with contract requirements and accreditation standards.
Conduct individual and group psychoeducational sessions for individuals scheduled for release, while coordinating medication and medical needs, including residential placement.
Establish and maintain relationships with community agencies, assisting with eligibility applications for financial assistance (SSDI, SSI) and resumption of benefits.
Collaborate effectively with Facility Staff, Parole Officers, and mental health and medical staff, ensuring confidentiality and adherence to security regulations.
Document services in health records, assist with referrals to healthcare providers, and perform additional duties as assigned.
