About The Position

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. To work as part of the interdisciplinary team to provide continuity of care support to the OP department as well as crisis intervention, support, safety plans and linkages to children, youth and families presenting to PATH for intake or crisis assessment.

Requirements

  • Bachelor’s Degree in the Human Services field from an accredited college/university
  • One to two years' experience in the behavioral health field
  • Good interpersonal and organizational skills

  • Experience working with children and adolescents preferred

Responsibilities

  • To provide crisis management that is coordinated and integrated with the Interdisciplinary Treatment Team of the UCC.
  • To provide telephone and triage coverage during scheduled hours to arrange for assessment, make referrals to appropriate services, and provide education and support of ongoing care.
  • Assist the family in understanding the events that led to the crisis from everyone’s perspective.
  • Assist the intervention specialist & Certified Psychiatric Nurse Practitioner or Psychiatrist in provision of crisis intervention, management, parent education and treatment interventions.
  • Utilize approved evidence-based tools and practices including SAMHSA crisis guidelines.
  • Utilize American Society of Addiction Medicine (ASAM) treatment criteria with youth who need substance use services.
  • Assist in developing a plan with input from the treatment team, for each individual and /or family member assessed. The plan will include ongoing follow-up, as indicated, beginning the day after the assessment to ensure that the process of linkage and stabilization has been initiated.
  • To assist in mobilizing resources and both internal PATH services & community services and supports, as needed &
  • Follow up within 7 days and 30 days to ensure linkages & promote ongoing services
  • Assist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation and education.
  • To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed and the plan has been developed.
  • Assure appointments are scheduled at a time and place convenient for the family.
  • To complete or assist others in completing involuntary commitment petitions.
  • To arrange and follow through with voluntary or involuntary hospitalization admissions.
  • To follow all outlined procedures regarding the initiation of a commitment.
  • To assist in developing and utilizing a linkage support network with citywide substance use, mental health, intellectual disability and other social service agencies.
  • To provide liaison services with CMHC’s as well as other specialized agencies.
  • Coordinate linkage to CRC at close of UCC, as needed.
  • To adhere to your schedule unless approved by a supervisor
  • To complete daily case conference as assigned
  • To provide crisis coverage support for OP therapists as needed
  • Participate in trainings and on-going discussions regarding the norms and values within the variety of ethnic and cultural groups in Philadelphia.
  • Obtain language/translation services as needed to work effectively with youth and families.
  • Complete all paperwork in a timely, comprehensive, legible manner and in accordance with PATH policies and procedures.
  • Attend all meetings related to assigned children/youth, as well as other assigned meetings.
