About The Position

CRISIS ACCESS SERVICES SPECIALIST - GRAND TRAVERSE MENTAL HEALTH CRISIS & ACCESS CENTER This role centers on providing immediate, compassionate care and clinical support to individuals experiencing mental health crises. Working within NLCMHA's 24/7 Crisis Walk-In Center and delivering mobile services in the community as appropriate, the Crisis/Access Specialist will ensure timely assessment, stabilization, and linkage to ongoing services. The position also includes assuring the following functions are carried out: assessment, determining medical necessity for ongoing services, planning, linking, coordination, monitoring, brief solution focused therapy and advocacy. This position reports directly to the Grand Traverse Mental Health Crisis and Access Center supervisor and/or the director. In order This position has no direct supervisory responsibilities.

Requirements

  • Credentialed by NLCMHA to provide services in this professional discipline.
  • Must have licensure with the State of Michigan as a fully licensed professional: Licensed Bachelor Social Worker. Must maintain licensure.
  • Bachelor Social Work from an accredited university required.

Responsibilities

  • Provide clinical services to individuals, primarily at the Grand Traverse Mental Health Crisis and Access Center. Direct services are expected to approximate 60% of the normal workweek through completion of crisis contacts including screens, inpatient assessments, access services including access screens and initial clinical assessments. Service location could be inconclusive in the community, ED’s and jails.
  • Provide indirect services, including case consultation and coordination, advocacy, and referral.
  • Will be required to complete inpatient screens for those at the Traverse City Munson ED whether in person or virtually.
  • Provide Brief Solution Focused therapy including groups. As qualified.
  • Participate actively in staff development activities including unit meetings, supervisory meetings, community meetings, applicable conferences, and review of current literature regarding services.
  • Work cooperatively with jail staff, Primary Care Physicians, local Courts, Family Independence Agencies, Schools, ISD, Substance Abuse and other providers to promote provision of quality community-based services.
  • Work cooperatively with other agency staff and different programs to insure provision of quality services.
  • Participate actively in Utilization Review, other Quality Assurance programs, in periodic evaluation of service output, impact of service delivery, and program planning.
  • Complete documentation of assessments, person-centered plans, review of compliance with documentation and contract requirements, quarterly reviews, and terminal evaluation summaries within time frames prescribed by unit policy in accordance with Medicaid and/or other third-party payer standards (including managed care).
  • May participate in interventions, assessments, and other aspects of crisis services during and after hours, as necessary.
  • May assist with completion of access screens and crisis assessments across counties.
