About The Position

The DFPS Cook Children’s Liaison will work with a multi-disciplinary, integrated healthcare team dedicated to serving children involved in DFPS cases who receive healthcare at Cook Children’s Medical Center. As a member of the multidisciplinary team, the Liaison will facilitate access to pertinent and timely information from a child’s DFPS, Community Based Care, and child placing agency caseworker to the child’s healthcare team. The Liaison will work collaboratively with DFPS staff, stakeholders, and healthcare professionals to promote optimal outcomes in child safety, child health, healthcare access, child well-being, education, and permanency. The Liaison performs advanced consultative services and technical assistance work; and works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Position will require in-depth knowledge of DFPS policies and best practices and the ability to become familiar with pediatric healthcare. The position interacts routinely with children, placement families, representatives from various organizations, Texas Department of Family and Protective Services, and the general public. This position reports to Child Protective Investigations Program Director Jessica Granado

Requirements

  • Skills in effective verbal and written communication.
  • Skills in establishing and maintaining effective working relationships.
  • Ability to operate a personal computer and use various software packages including the “Information Management Protecting Adults and Children Systems” (IMPACT) and Microsoft Word, Excel and PowerPoint.
  • Ability to travel and attend community meetings and case staffings.
  • Ability to work in an emotion-filled, fast-paced environment.
  • Ability to provide presentations to various groups about the integrated healthcare needs of children in foster care and those involved in DFPS cases.
  • Ability to multi-task and create a well-designed organization system to track assigned tasks and progress
  • Applicants for positions which require, as part of the essential functions of the job, driving a motor vehicle, must have a reliable motor vehicle, and acceptable driving record for the past five years, and a current, valid Texas driver's license appropriate for the vehicle and passenger or cargo load. Applicants must provide proof of driving record and license.
  • Employed as a Child Protective Services Specialist III with the Texas Department of Family and Protective Services for at least 6 months OR currently employed as a Child Protective Services Specialist IV with the Texas Department of Family and Protective Services OR currently employed as a Child Protective Services Supervisor I or II with the Texas Department of Family and Protective Services OR currently employed as a Child Protective Services Program Director or Child Protective Services Program Administrator with the Texas Department of Family and Protective Services.

Responsibilities

  • The Liaison communicates information to inform the team in development of optimal preventive, medical, educational, therapeutic, and supportive treatment plans. This includes case information that affects health and safety, healthcare delivery, or child well-being, in order to provide wrap-around care.
  • The Liaison will facilitate access to pertinent and timely information from a child’s healthcare team to Child Protective Investigations and Child Protective Services to provide comprehensive health information that ensures a child’s needs are being met as related to safety, well-being, permanency planning and transitions.
  • The Liaison works closely with Nurse Coordinators for early identification of issues related to access to care, placement compliance with treatment plans, placement disruptions, or transitions of children with special healthcare needs. The Liaison will also be available for discharge planning of patients of the Foster Care Clinics.
  • Documents information in the electronic medical record, as permitted by Cook Children’s Medical Center, and provides documented information to the CPS caseworker for inclusion in the CPS record.
  • Develops and maintains effective working relationships between Child Protective Investigation staff, Child Protective Services staff, law enforcement officials, judicial officials, legal resources, medical professionals, CASAs, CPAs, and other community resources.
  • Serve as an expert within Cook Children’s Medical Center to improve hospital policies, procedures, and coordination of care affecting children in DFPS cases.
  • Serve as an educator, in collaboration with members of Cook Children’s Medical Center, with community stakeholders, child placing agencies, CASAs, foster parents, peers in DFPS, and healthcare professionals.
  • The Liaison will also assist in resident education of DFPS processes and child abuse reporting.
  • Attends and participates in multidisciplinary team case review.
  • Performs other duties as assigned by the CPI Program Director and Cook Children’s Medical Center Director
  • Promotes and demonstrates appropriate respect for cultural diversity among coworkers and all work-related contacts.
  • Attends work regularly in accordance with agency leave policy.
