As a Case Manager, you will work with various providers in the region or system to facilitate access to services. You will be well versed in resources available in the region and support the No Wrong Door philosophy. Your role will involve assisting clients in identifying their housing goals, creating an action plan to achieve such goals, and accessing housing resources needed to achieve their goals. You will facilitate housing preparation education for clients addressing issues necessary to maintain housing and a higher quality of life. Additionally, you will provide updates to the treatment team on clients' options for available housing and facilitate community appointments and meetings to assist clients with achieving their goals. This includes coordinating with community supports, transporting clients to and from scheduled appointments, and being an advocate for clients' self-determination. You will assist clients in developing client service plan goals, prioritizing areas of need, and steps needed to reach established goals, while continually monitoring progress towards these goals and supporting clients' efforts toward self-sufficiency. You will also conduct triage and walk-in services, care coordination, and/or crisis intervention as requested by direct supervisors, and provide crisis interventions as needed to ensure the safety of clients, other members, and staff. This position requires being eligible and able to drive company vehicles and/or transport clients in your own personal vehicle to provide field-based services.
