About The Position

As a Counselor I - Case Manager, you will provide case management, rehabilitative, psycho-social education, and other type services via milieu community settings. You will assist clients and families to build upon natural support systems and link individuals to other service agencies. Your role will include providing additional supportive services to assist with continuity of treatment, linkage to resources, transition from program to program, and transition from program to community resources. You will connect every client/family to a medical home as well as clinic, home, and community-based services, serve as an advocate for the client and family, and promote healthy development and positive outcomes for clients through utilizing case management and rehabilitative strategies with a team of dedicated professionals. Additionally, you will coordinate monthly social gatherings for residents, create and maintain all required documents in the electronic and paper record systems, and support the Program Director and Assistant Program Director with authorizations and intakes.

Requirements

  • Bachelor's degree in a mental health-related field such as Psychology, Social Work, or Counseling required.
  • Minimum of 1 year of experience working with adults and older adults with mental health challenges.
  • CPR/First Aid certification is required and must be maintained during employment.
  • Ability to thrive in a fast-paced environment is essential.
  • Proficient computer skills, including MS Office.
  • A valid California driver's license in good standing and eligibility to be an approved CRF driver is required.
  • Ability to pass criminal/background clearance.
