About The Position

LSSI is hiring a full-time Counselor Case Manager for the Elgin Residential Rehabilitation Program. Provide whole-person services to clients participating in the Elgin Residential Rehabilitation Program, including 1:1 sessions, case management assistance, and the presentation of evidence-based group programming. Assist clients in achieving their stated goals and objectives, collaborating with other internal and external treatment professionals.

Requirements

  • High school diploma or GED required.
  • Associates, Bachelors or Master's degree preferred.
  • 1-3 years of experience in substance use field is preferred.
  • CADC or must obtain CADC within 18 months of hire.
  • Annual TB test is required.
  • Familiarity with accreditation and licensure standards.
  • Proficiency in EHR documentation and review.
  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office - Outlook, Word, Excel and PowerPoint) along with other related software.
  • Excellent written and verbal communication skills.
  • Ability to work in a team environment.
  • Valid Illinois Driver's license.

Responsibilities

  • Collaborate with Elgin Residential Rehabilitation team members, other LSSI programs, and external partners in order to provide quality, whole-person care and services to Elgin Residential Rehabilitation clients.
  • Maintain a caseload of 5-8 clients.
  • Present weekly groups, founded in evidence-based practices.
  • Provide weekly 1:1 counseling to each client on assigned caseload.
  • Provide case management services, including appropriate linkages and referrals.
  • Provide support coverage to Elgin Healthy Start Recovery Home clients, as needed.
  • Complete all required EHR documentation in a timely manner.
  • Meet productivity requirements identified by LSSI.
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
