About The Position

In an environment of continuous quality improvement, the COPD Navigator will execute all necessary action to reduce/eliminate exacerbations and slow COPD related respiratory clinical decline in patients. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times. Job Functions: Assists in development and implementation of quality improvement processes for the management of COPD patients across the care continuum Identifies care gaps and facilitates closures Monitors and tracks all COPD readmissions Identifies and implements measures to reduce COPD readmissions Consults and collaborates with the multi-disciplinary team to coordinate patient plan of care Verify patients understanding and ability to properly use home medications Recommend medications and treatment pathways based on GOLD guidelines Perform patient needs assessment to identify and address barriers to care Participate in the development of standardized processes and procedures that support efficient and effective continuity of care practices Connects patients to key Pulmonary services Educate patients, family, and providers on COPD care Provides self-management education and champions individual care plans Assumes all other duties and responsibilities as necessary.

Requirements

  • Registered Respiratory Therapist
  • Licensed by the state of Ohio with at least 7-years’ experience as a Respiratory Therapist required.
  • Completion of the AARC Pulmonary Disease Educator Course within 3 months of hire required.
  • Thorough understanding of GOLD Guidelines.
