About The Position

The main purpose of the Coordinator Special Programs Health Services II, Mental Health position is to provide intensive street-based case management. This position will provide care coordination, follow-up and resource linkage to individuals referred to the Law Enforcement Assisted Diversion (LEAD)/Quick Response Team (QRT) program.

Requirements

  • Graduation from an accredited high school or possession of a high school equivalency certificate.
  • One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
  • Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

  • Experience providing behavioral health case management services.
  • Experience using a nonjudgmental, person-centered approach when engaging with clients.
  • Experience applying trauma-informed approaches in case management.
  • Experience/knowledge of the American Society of Addiction Medicine (ASAM) levels of care.
  • Experience maintaining routine and confidential files, logs and program data.
  • Experience organizing and completing work according to assigned schedules.
  • Experience working independently and with groups.
