Contracted In-Home Occupational Therapist
About The Position
We’re looking for an Occupational Therapist who is passionate about helping older adults and individuals with disabilities thrive safely and independently at home. A successful candidate is highly empathetic, can juggle the needs of our clients, programs and business, and is eager to make a lasting impact. We’re a tech-first team so comfort working with new systems and tools will be an advantage. Also a huge plus if home modification work is part of your experience. Requirements
- Degree in Occupational Therapy from an accredited program
- Active, unencumbered OT license
- 2+ years of clinical experience
- Strong interpersonal and clinical reasoning skills
- Reliable transportation and valid driver’s license
- Proficiency with technology and digital documentation
- Ability to work independently while collaborating with a remote team
Nice To Haves
- Experience in home health, aging in place, or disability services preferred
- home modification work is part of your experience
Responsibilities
- Conduct in-home assessments and functional evaluations
- Deliver personalized, evidence-based recommendations for home modifications
- Educate clients and caregivers on fall prevention and safe mobility techniques
- Engage clients and families in collaborative care planning
- Work closely with our Director of Clinical Operations to share findings and support process improvement initiatives
- Maintain accurate and timely documentation through our electronic system
