Requirements
- Current unrestricted Occupational Therapist license in state for which care is being provided
- One (1) year experience as occupational therapist, preferably in an in home setting
- CPR Certification, required
- Two-step negative TB skin test or chest x-ray, required.
- Level 1 Fingerprint Clearance Card, required
- Valid Driver’s License and auto insurance, required
- Understands regulatory compliance with state and federal regulations
- Adheres to HIPAA and maintains client confidentiality
- Transportation to meet the needs of position and scope of job responsibilities, required
Responsibilities
- Comply with all state-mandated Occupational Therapy Practice Act, Occupational Therapy Code of Ethics and other legal mandates.
- Evaluate client for therapy needs and provide written orders/treatment plan for services.
- Ensure plan of care is consistent with client assessment and needs.
- Instruct other personnel and/or family members and caregivers on intervention goals.
- Ensure proper visit documentation completion.
- Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider.
- Participate in discharge planning..
- Provide in-service education for agency personnel, as needed.
- Ensure personal credentials are active and up to date at office.
- Attend all staff meetings and complete all required annual education as assigned.
