About The Position

The Consumer Services Coordinator is responsible for ensuring that program residents receive high-quality services within the WellLife Network Ann Mittasch Intermediate Care Facilities (ICF) residential program and the Ann Mittasch Day Habilitation program. This individual must have a working knowledge of ICF and Day Habilitation documentation, including persons served goals, Comprehensive Functional Assessments (CFAs), Life Plans, and must be proficient in the use of computer systems, including the electronic health record (EHR) and other relevant applications (i.e., Outlook, Microsoft Office, Microsoft Excel, etc.).

Requirements

  • Bachelor’s Degree in Human Services required.

  • Three to five years’ experience in DD services and demonstrated proficiency in DD residential setting is preferred.

Responsibilities

  • Ensures the development, implementation, and recording mechanisms for provision of active treatment to persons served are aimed at increasing each person’s capacity to function independently.
  • Serves as Qualified Intellectual Disabilities Professional (QIDP) as needed and oversees the services to the persons served of assigned sites in accordance with agency policies and procedures and OPWDD regulations and guidelines.
  • Provide management oversight and required duties to an assigned location in the absence of a manager.
  • Review goals and services in the EHR to ensure they appropriately address recommendations from the IDT, and specific persons served needs.
  • Review goal progress in the EHR to ensure progress, stasis, and regression are acted upon accordingly.
  • Conducts monthly internal audits of ICF program and Day Habilitation Program records (within the EHR and program binders), including goal progress, goal turnover, annual and semi-annual CFA’s, Life plans, and monthly summaries and IDT meeting minutes, and identifies any problem areas.
  • Ensures that Personal Expenditure Plans (PEP) and Money Management Assessment (MMA) are completed and updated as needed in the ICF.
  • Ensure that Staff action plans and Day Habilitation monthly summaries are completed in accordance with OPWDD standards.
  • Communicate information from reviews to the Residence Manager, Day Habilitation Program Coordinator, Director of Day Habilitation Services, and Division Director(s) of DD Services.
  • Ensure the programs operate in accordance with agency, state, and federal standards.
  • Review all Plans of Corrections (POCA’s) to ensure the QIDP component is met as always needed and ensure audit readiness.
  • Ensures assigned locations are audit ready based on WLN expectations for audit performance.
  • Required to display proficiency while entering person served data documentation and reviews in the EHR system (Electronic Health Record).
  • Assist in observations and training of staff to ensure persons served needs are met, and service plans are implemented appropriately.
  • Ensure the provision of training is in accordance with specific person’s served concerns and/or needs, such as special protocols.
  • Participate in IDT meetings for all persons served at the Ann Mittasch ICF and Day Hab location to ensure needs are met through an appropriate plan of care, ex: complex or high-profile cases.
  • Review for accuracy the Life Plans, CFA/Annual, Semi-Annual, and Special IDTs.
  • Review plan of care and ensure all recommendations are implemented in a timely fashion.
  • Assist in training of Residence Managers / Site Supervisors, on how to complete the CFA’s, IDT meeting minutes, PEP’s and MMA’s, goal writing, goal progress, and goal turnover, as needed.
  • Assist in training Day Habilitation Program Coordinator, Assistant Habilitation Specialist, and Habilitation Specialist as needed.
  • Serve on standing committees as assigned by supervisor, i.e., Human Rights, Incident Review, Admissions, Infection Control, Persons Advocacy committee.
  • Must be able to physically interact with the consumer, negotiate the environment independently, including assisting in all SCIP interventions, emergency CPR, First Aid, and fire drills/evacuation.
  • Assist management teams in updating persons served records in the selected EHR (Electronic Health Record), including but not limited to completion of the daily census, review of goal data collection, etc.
  • Available for assistance in emergencies, including on-call, early morning, evening, or weekend responsibilities.
  • Participates in the AOD (Administrator on Duty) rotation.
  • Work schedule and location subject to change, including early morning, evening, or weekend responsibilities, based on agency or departmental needs.
  • Maintain confidentiality at all times.
  • Other duties as requested based on the needs of the facility and/or agency
