About The Position

Provide consultation services to participants and/or their representatives accessing Community First Services and Supports (CFSS) through a person-centered approach that promotes self-direction and reflects the core Principles of Self-Determination. This includes supporting individuals to make meaningful decisions about how they want to live their lives, exercise authority over a designated budget, and organize resources in ways that enhance their quality of life. Responsibilities also include fostering accountability in the use of public funds, recognizing the valuable contributions individuals with disabilities bring to their communities, and affirming the essential role of self-advocates in shaping and navigating a redesigned support system. Services include CFSS orientation and training, ongoing case management, assistance with service plan development, and facilitation of connections to external resources and service providers.

Requirements

  • High school diploma and a minimum of one year of full-time experience providing direct services to individuals with disabilities and/or adults age 65 and older.
  • Experience coordinating or directing services for individuals with disabilities or older adults, including experience with self-directed service models.
  • Demonstrate willingness and ability to travel 20–35% of the time to engage with participants, their representatives, state and lead agency personnel, and to attend external trainings.
  • Possess a valid driver’s license, maintain a clean driving record, and have access to reliable transportation for work-related travel.
  • Demonstrate proficiency in the Microsoft Office Suite and strong general computer skills.
  • Exhibit strong analytical skills and financial literacy.
  • Demonstrate strong verbal and written communication skills.
  • Bring in-depth knowledge of Minnesota’s Long-Term Services and Supports, with a specific focus on Disability and Aging services and associated funding sources.
  • Demonstrate a commitment to the Principles of Self-Determination and person-centered service delivery.
  • Are able to pass both the State of Minnesota Department of Human Services background study and a drug screening.
  • Have experience working with intersectional and diverse identity groups.

Nice To Haves

  • Bachelor’s degree in a human-services–related field preferred. Relevant fields may include, but are not limited to, disciplines focused on human behavior, skill development, cultural studies, rehabilitation, or the arts as they relate to human care and support.
  • Personal experience coordinating one’s own services within a self-directed model preferred.
  • Case management experience related specifically to Minnesota’s Long-Term Services and Supports preferred.

Responsibilities

  • Demonstrating consistent mastery of grammar and a broad vocabulary to clearly express thoughts, ideas, and actions; excelling in structuring and conveying complex concepts to diverse audiences; actively listening and probing for deeper understanding; and consistently summarizing intricate ideas and situations with accuracy and clarity.
  • Performing at a high level in dynamic environments while serving as a proactive advocate for change; skillfully balancing the need for consistency with the benefits of adapting behavior to evolving circumstances; and applying a deep understanding of political, commercial, and practical factors that influence change to anticipate impacts and guide appropriate responses.
  • Serving as a subject matter expert across all Lifeworks service offerings with a comprehensive understanding of service structures and access pathways; and providing mentorship to colleagues seeking to deepen their knowledge of Self-Directed and In-Home service lines, as well as the competitive landscape in which Lifeworks operates.
  • Benchmarking best practices from within and outside the industry to inform organizational improvements; participating in professional conferences; maintaining relevant certifications; and being recognized as an expert within the organization and potentially across the industry, often holding leadership roles in professional associations and maintaining superior credentials in the functional area.
  • Advocating passionately for individuals’ desired outcomes, even when facing resistance from extended support teams; treating individuals with respect; achieving optimal outcomes by identifying and leveraging strengths and preferences; and serving as a resource for colleagues in recognizing and supporting individual strengths and goals.
  • Building and sustaining strong, authentic, and mutually beneficial relationships across diverse styles and backgrounds; expanding networks proactively through shared interests and goals; transforming challenging relationships into productive ones, especially during difficult circumstances; and prioritizing internal and external customer needs while defining team requirements and making sound decisions that balance short- and long-term outcomes.
  • Performing other duties as assigned.
