The Connections Coordinator is responsible for managing outreach, screenings, admissions, and referral coordination for all MHFV programs. This role ensures compliance with funding, regulatory, and organizational requirements while serving as a primary point of connection between participants, treatment providers, and community partners. The coordinator plays a critical role in assessing participant needs, facilitating access to services, and supporting continuity of care through a trauma-informed, client-centered approach.

Requirements

  • Bachelor’s degree in social work, Counseling, or Human Services, with experience in a substance abuse treatment environment preferred. CARES/CPS certification is also acceptable.
  • Tech savvy; Proficiency in Microsoft Office Suite and other relevant software.

Responsibilities

  • Complete initial screenings for all MHFV programs in accordance with agency policies and funder requirements.
  • Conduct comprehensive biopsychosocial assessments to determine eligibility, clinical needs, and appropriate program placement.
  • Collect and document psychosocial history, including substance use, mental health, medical, housing, legal, vocational, and family systems.
  • Ensure the Connections Department operates in compliance with all funding sources and regulatory bodies (DBHDD, CARF, SAMHSA, VA, HUD, etc.).
  • Monitor electronic health records weekly to ensure accurate, complete, and timely documentation.
  • Maintain working knowledge of MHFV connection and clinical policies; ensure adherence and assist with updates as needed.
  • Conduct outreach activities and connect women to housing, treatment, peer support, case management, and wraparound services.
  • Manage and maintain Memorandums of Understanding (MOUs) with collaborating partners.
  • Identify and develop community resources to strengthen collaborative partnerships and service delivery.
  • Track and document performance measures, including agencies visited, referrals received, screenings completed, and unduplicated enrollments.
  • Conduct site visits to treatment providers and coordinate with wrap-around service agencies.
  • Collaborate with clinical and program staff to support coordinated service planning and continuity of care.
  • Ensure compliance with HIPAA, 42 CFR Part 2, and all organizational policies and procedures.
  • Participate in team meetings and contribute to continuous quality improvement of the admissions and connection process.
  • Provide crisis intervention and immediate referrals when needed during screenings or assessments.
