As a Connecting Communities Case Worker with Change Grow Live you’ll work with people in custody, who use our services, to prepare for transition back into the community.This is an excellent opportunity for someone with experience of substance misuse issues, looking to take the next step in their career with a charity that will support your continued personal and professional development. This role is based across 4 prison sites.

Requirements

  • Excellent understanding of drug and alcohol issues and experience of working within a related field.
  • A commitment to multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group, e.g. mental health services, criminal justice agencies, community groups, etc.
  • Accomplished written and verbal communication skills and a high degree of personal IT competency. The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information, e.g. reports.
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
  • Experience of working within a prison/criminal justice setting
  • Sensitivity in working with complex and diverse cultural issues

Responsibilities

  • Change Grow Live currently provide the psychosocial interventions and transitional services across prisons in Kent. This includes the Connecting Communities Team. This team will prepare people engaging with CGL and Oxleas for release back into the community.
  • Dedicated staff will work together with healthcare colleagues supporting clients when transitioning to communities across the UK. Our enhanced offer will include flexibility to engage with clients for a period of time post release.
  • We are looking to recruit a passionate and committed individual to the role of Case Worker within the Connecting Communities Team based within the following four prisons:
  • HMP Rochester is a category C adult's male prison.
  • HMP Cookham Wood is a category C adult's male prison.
  • HMP Maidstone is a category C training prison for foreign national prisoners.
  • HMP East Sutton Park is a women's open resettlement prison for adults and young women
