About The Position

Welcome to a medical center where you're the center of attention. Pay range: - Hourly $51.79 - $77.69 Overlake has an outstanding opportunity for a Complex Nurse Case Manager to join our team. The position is full-time onsite, Monday-Friday. Schedule: Monday - Friday, Day Shift This position is responsible for caseload of complex patients with longer length of stays across multiple hospital units. Responsibilities include: conducting psychosocial assessments, facilitating and coordinating post-acute services for patients, including Skilled Nursing Facility, Acute Rehab Unit, Home Health, Hospice, Adult Family Home, Assisted Living, as well as other discharge dispositions, and working closely with Transitions of Care Director and Medical Director as well as with providers and the multidisciplinary team members, to problem solve and mitigate barriers to progress complex discharge plans. Qualifications: Associate degree in nursing required. Bachelors or Masters degree in nursing preferred. Current Washington State license as a Registered Nurse required. Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred. Five years acute care nursing experience required. Two years inpatient case management experience preferred Prior case management, utilization management, and/or payer experience required. Ability to effectively present information and respond to questions from groups of managers, patients, physicians, and payors. Effective verbal and written communication skills and computer skills. Requirements

  • Associate degree in nursing required. Bachelors or Masters degree in nursing preferred.
  • Current Washington State license as a Registered Nurse required.
  • Five years acute care nursing experience required.
  • Prior case management, utilization management, and/or payer experience required.
  • Ability to effectively present information and respond to questions from groups of managers, patients, physicians, and payors.
