About The Position

This position is responsible for the coordination and support of Complex Care Patients. The primary aim is to identify and evaluate patients who present with complex medical and/or psychosocial needs. The goal of the coordinator role is to support the interdisciplinary team with coordinating services through community linkage to the appropriate placement to promote health and healing.

Requirements

• Bachelor's degree in Social Work, Sociology, Psychology, Public Health, Human Services, or related field is required.
• Two (2) years of experience in community case management is required.
• Extensive knowledge of local community resources (Fresno/Clovis and outlying areas) is required.
• Experience working with individuals, community systems, and with multi-disciplinary teams is required.
• Current American Heart Association (AHA) Healthcare Provider CPR card is required.
